Register for ESOBA

Application form

The application form must be completed by the Training Manager and returned to etdtqi@nhs.net

Training on ESOBA

A member of the TQI team will contact you to arrange a 30 minute training session. This will be conducted online using Adobe ConnectNow or WebEx which will allow you to view the application remotely.

Please note: the training manager can ask another member of the training team to complete an online asessment on their behalf.
