Application Form
Use this application form to apply for the IG SoC process, in particular if you are seeking approval for a new connection to the N3 network or other NHS Connecting for Health systems and/or services.
NB GPs must not complete this application form. Please see the separate page dedicated to GPs.

- Offshore Support Requirements (PDF 53Kb) - Please provide a full response to each requirement indicating how your organisation complies with each.
- Information Security Management System (ISMS) Template (MS Word 127Kb) - Please complete this document as part your answer to Requirement 9 of the Offshore Policy (link above)
- Information Governance Toolkit Improvement Plan Template (MS Excel 31Kb) - You will need to complete this if you have submitted your IG Toolkit assessment but have not achieved the required level on any requirements. The IG Toolkit team will inform you of this requirement once your submission has been assessed.
- Information Governance Assurance Statement v2.0 (PDF 40.2kb) - For information only, this statement replaces the previous IG Statement of Compliance, and is accepted as part of the IG Toolkit submission.
Changes to existing N3 connections
Organisations wishing to change the configuration of their N3 connection, including amendments to I.P Addresses e.g. to allow third party access to their network, please complete a Filter Request Form (Word 37Kb) and submit to btn3ccc@bt.com
