Leaflets

To support local communication activities a number of NHS Number leaflets targeted at both staff and patients have been produced.  The web versions of these leaflets are downloadable. There are also print versions of these leaflets available so these can be printed locally to support any communication activities your organisation is undertaking.

Currently these leaflets are also available to order, simply enter the leaflet reference number and follow the instructions.

Trusts have also produced covering letters when distributing these leaflets.

Staff Leaflet (ref 4433)

This leaflet is the current information leaflet for NHS staff. 

Your NHS Number (ref 4249)

The NHS Number Patient Information Leaflet provides information on how to find your NHS Number and has space on the reverse of the leaflet to include the NHS Number.  Organisations can print out a patient's NHS Number and other demographic details on sticky labels and place in the space provided.

This leaflet has also been translated into a number of alternative languages.

From Birth for Life (ref 4369 & 4369 LP)

The NHS Number from birth to life leaflet provides information for parents on the importance of the NHS Number and provides details on how to find the NHS Number for their baby.  The leaflet has a space on the reverse to include the babies NHS Number.  This leaflet is also available in large print.

Supporting letters

Trusts have also developed covering letters for distribution with the patient leaflets to GP Practices and Provider Services (Secondary Care).   The letters explain why the leaflets are being used and request that the leaflets are displayed &/or handed out to patients.
