Patient Experiences
Helping Colin get the right treatment when away from home (4728, PDF 172Kb) - A former VAT consultant, Colin's health began to deteriorate in 1998, when he had problems with his breathing. A keen holiday maker and taking 15 types of medicines, Colin explains how the SCR can help him when he is away from home.
Knowing which medicines Tracey is taking when in an emergency (4727, PDF 175Kb) - Following a traumatic experience, Tracey was diagnosed with type 1 diabetes in 1996. Tracey tells of how the SCR has proved vital during times where she has required urgent care.

Community and GP Practice Care Settings Experiences

Hospital Pharmacy Experiences
Medway Maritime Hospital has approximately 150 patients admitted for hospital care and treatment every day. The introduction of Summary Care Records (SCRs) at Medway NHS Foundation Trust has improved medicines reconciliation for patients admitted to wards.
