SCR Public Information Programme (PIP) Toolkit
Summary Care Records can be made for patients once GP practices and PCTs have undertaken staff engagement and patient/public awareness activity through a Public Information Programme (PIP). A range of guidance, materials and resources are available to support the NHS to manage their Summary Care Record Public Information Programmes (PIP).

PIP information pack
The PIP information pack sent to patients includes:
- PIP template patient letter (Word, 245Kb) A template patient letter introducing Summary Care Records, for local tailoring. Only sections highlighted in yellow are editable.
- A5 Patient summary leaflet (ref: 4706) - Web version (PDF, 232Kb), Print locally version (PDF, 1.16Mb), Print version with crop marks (PDF, 1.24Mb) A patient leaflet explaining Summary Care Records.
- Opt-out form (PDF, 515Kb) The form patients need to fill in if they do not want a Summary Care Record. This form is also available in large print A4 (PDF, 221.6kB) (PDF, 221.6kB). (Download only)
